Normalization of Deviance in Manufacturing

 "We’ve been doing it this way for a long time, and no one said anything."


That is one of the most dangerous sentences in a GMP and Health & Safety environment.

Normalization of deviance does not announce itself. It builds through small, unchallenged exceptions until they become the culture.

You hear it in comments like:

“QA hasn’t flagged it.”
“I don’t want to open a can of worms.”
“It’s not impacting output.”
“Regulatory authority did not catch it last time.”
“We’ll fix it during shutdown.”

These are not casual comments. They are early warning signs of a culture drifting away from standards.

What strong cultures do differently:
• They follow procedures. If something does not work, fix it through change control. No workarounds.
• Avoid assumptions. They verify, document, and act on facts (data-driven decision making)
• Prevent recurrence. Use effective CAPA to address root causes, not symptoms (Poka-Yoke, mistake proofing).
• Build psychological safety so anyone can speak up without fear of consequences.
• Improve daily, not just before audits (Kaizen + Gemba walks).
• Automate where it reduces human error, improves consistency, and strengthens compliance (Jidoka + Poka-Yoke).

Right First Time is not a slogan. It comes from the decisions made when no one is watching. Fix the gap before it escalates into something bigger. Or worse.
